Who is eligible for home health care?
Trained nurses and therapists assist patients and caregivers with recovery from illness or surgery. They can also provide assessment and teaching to help patients manage their diseases at home and prevent hospitalizations. Care is individualized to each patient’s needs.
Services are provided to homebound patients who need intermittent, skilled services. Read on for specific details about how these terms apply.
Homebound: This term does not mean that a patient is strictly confined to bed or a wheelchair or even to their homes; it also applies to patients who can leave their homes for medical appointments or infrequent outings, however, require assistance from another person or device to do so. Patients who are driving are not considered homebound.
Skilled services: This term refers to care by a trained, professional nurse or therapist with the appropriate certifications. Care must be considered reasonable and necessary. While a home health aide can provide personal care, this can only occur if the patient also requires the care of a skilled provider. The agency does not offer home-maker or companion services, although sitter lists are provided to patients on request.
Intermittent: This means nurses and therapists visit a few times a week or less often, as ordered by the physician to monitor the patient’s progress and intervene when necessary.
How is home health paid?
Benefits and eligibility are reviewed case by case when a referral is received before admission. At that point, we determine coverage of services to ensure accurate billing.
