Who is eligible for hospice care?

People with an illness causing limited life expectancy may receive hospice care. The patient’s clinical information must support a life expectancy of six months or less if no further aggressive treatment is provided. However, some patients may live longer than six months. Hospice care is appropriate as long as a review of the patient’s condition supports limited life expectancy.

Hospice care is designed for patients and families who have chosen to focus on symptom and pain management, not curative treatment. Hospice care does not mean less care, only that the focus shifts from curative to supportive.

Hospice is frequently associated with cancer, but there are many chronic illnesses and conditions in which a patient is considered to be terminal with a limited life expectancy. Patients who exhibit weight loss, functional decline, decreasing ability to eat or drink, frequent hospitalizations, chronic infections, worsening breathing problems, pain exacerbations, or a combination of various symptoms may qualify for hospice support.

Diagnoses commonly associated with hospice care include end-stage phases of:

  • Cancer

  • Alzheimer’s dementia

  • Neurological diseases

  • Liver disease

  • Heart disease

  • Lung disease

  • Kidney disease

  • Stoke or coma


Where is hospice care provided?

Care may be provided in a patient’s home, assisted living facility, or nursing home. Hospice may also be provided in a skilled facility or hospital for those patients requiring a high level of care that cannot be provided in the home setting.


Who provides the care?

Lincoln Medical Hospice utilizes a compassionate team of trained professionals and volunteers to individualize each patient’s care plan and provide services to meet that patient’s and family’s needs. The interdisciplinary team includes:

  • Medical director

  • Trained hospice nurses

  • Home health aides

  • Medical social worker

  • Bereavement coordinator

  • Chaplain & spiritual support

  • Trained volunteers

How is hospice paid?

Medicare, Medicaid, VA Benefits, or private insurance usually cover hospice. However, Lincoln Medical Home Health and Hospice is dedicated to offering the same quality care to residents of Lincoln County regardless of the patient’s ability to pay.
